MY2012 Registration is open to all boys & girls
ages 4 - 14
(birth dates between 8-1-98 and 7-31-08)
No soccer ability or experience required –
Everyone Plays!
Registration Fee – $45
Fall 2012 & Spring 2013
The registration fee is for each child.
This registration is for the Fall 2012 AND Spring 2013.
Cash or checks accepted payable to Danville AYSO.

If you are unable to attend one of these events, and it is
prior to May 31st, please mail two signed copies of these
forms and a check made payable to

After June 1st, the fee increases to $60 per player and we
can not guarantee a spot will be available in your child's

* All New Players - COPIES of Birth certificates must be included with your registration forms *
Uniforms may be purchased at an additional cost.
Uniforms can not be ordered in the eAYSO system.

Please Volunteer - We Need You!
AYSO is an "All Volunteer" run program. No one is paid for their work. We are always looking for motivated people interested in helping our program with all the various tasks it requires to run smoothly. Board Members, Coaches, Assistant Coaches, Team Referees, Team Parents, Field Day help, and Registration Day help are all vital to our continued success. NO PRIOR SOCCER EXPERIENCE IS NECESSARY for you to volunteer. AYSO has extensive training for all positions.
